Program Summary

Person Centered Planning is a formalized process in which the individual receiving supports and services is helped to lead the process of developing a plan of service. An evidence-based and strength centered practice, its is applicable for prevention and treatment settings.

It is founded in principles of working with the total person by studying them, what a person is good at, what the person can do and has done. It entails demonstrating compassion while “seeing” the world as the client sees it.

The client’s plan of services would include goals, objectives and interventions intended to ameliorate debilitating symptoms and conditions.
